The U.S. Occupational Safety and Health Administration’s (OSHA) Deputy Director, Directorate of Enforcement Programs Patrick Kapust recently unveiled the Top 10 OSHA violations of fiscal year 2021, which concluded on September 30. Announced on Oct. 12, 2021 during the 2021 NSC Safety Congress & Expo event in Orlando, Fla., this year’s listing has remained relatively unchanged.

NEC: Lights Out Doesn’t Mean the Switch is Off

It’s good to keep an understanding of switches in mind when working on industrial or commercial systems and equipment. Just because you flip a control switch off doesn’t mean the circuit is dead or an enclosure is de-energized. Lockout/tagout procedures for utilization equipment always take you back to the branch or feeder supply, which is a fuse or breaker. A similar concept applies to service equipment.

PREMIUM CONTENT: Russ found a big hole on the top of this panelboard enclosure. This is a very dangerous situation. If someone accidentally drops a tool, bracket, bolt, screw, etc. in this hole, it could generate a short circuit and possibly an arc flash. Section 110.12(A) in the 2020 NEC says that unused openings shall be closed to afford protection substantially equivalent to the wall of the equipment.
