On a recent trip to Las Vegas, I found this installation as I made my way back to the hotel from the convention center. This arrangement was in a parking lot behind a block wall. The damage appears to have been caused by a truck bumper from a nearby parking space. This equipment was still running and looks like it was installed without the benefit of an electrical permit and electrical inspection.

I’m sure the equipment ground has been lost due to the damage caused by a lack of physical protection. In addition, there’s no disconnecting means in sight. See 110.3(A)(8) of the 2008 NEC.
