The fault I see is an extension cord running behind a gutter.

At the present time I see no violation. Everyone else is speculating on what they see, so here it goes. It is a GFI receptacle for a heat tape that is keeping the gutter from freezing. At the time of installation it was most likely code for the cover. You could make a case for a strap within 36 inches or possible expansion joint. Just because we are working out of the 2011 NEC code book does not mean all work that was done previously to the newer code is not code.

Dave, you make a good point. Sometimes the installation was done in compliance with a previous edition of the code, but even without knowing WHEN the installation was made we can still use it to point out the violations of the current code.
