The special provisions for electric-discharge lighting systems of 1,000V or less are outlined in Art. 410, Part XII of the 2011 NEC. As noted in Sec. 410.130(G)(1), "In indoor locations other than dwelling and associated accessory structures, fluorescent luminaires that utilize double-ended lamps and contain ballast(s) that can be serviced in place shall have a disconnecting means either internal or external to each luminaire. For existing installed luminaires without disconnecting means, at the time a ballast is replaced, a disconnecting means shall be intalled. The line side terminals of the disconnecting means shall be guarded."
But there are five Exceptions noted for this rule. Exceptions No. 2, 3 and 4 are outlined in answers a, b and c above.
