"Section 410-56(f)(3) is violated. Receptacles mounted to, and supported by a cover raised or flat, such as shown in your photo, are required to be secured by more than one screw. Only one screw was used in the installation shown. This makes the reason for the rule obvious.
Also, there is no indication that the cover was listed, or identified for securing the receptacles with only one screw."
P.S.: Joe, I see no problem with the position of the grounding terminals being different, since there never was, or ever will be a rule in the National Electrical Code requiring consistency "Up or Down."
