Primecraft Doors in Malvern issued 32 citations including 27 serious

The Occupational Safety and Health Administration has issued 27 serious citations to Primecraft Doors in Malvern, Ark. The citations included electrical shock and electrocution hazards. OSHA also noted that proper procedures to lockout/tagout machines to prevent sudden machine starts or movement were not in place.

Five more citations were issued for improperly lit exits and not providing workers with required personal protective equipment. The proposed penalty is $65,600. OSHA began an inspection on Dec. 2, 2015, after a complaint about several unsafe working conditions.

"With more than 30 violations, Primecraft Doors must immediately address many hazards to the safety and health of their employees before someone gets seriously injured or worse," said William "Monty" Cole, OSHA's acting area director in Little Rock.

Headquartered in Hot Springs, Primecraft Doors Inc. is a custom wood door manufacturer. The company has 15 business days from receipt of its citations to comply, request an informal conference with OSHA's area director, or contest the citations and penalties before the independent Occupational Safety and Health Review Commission.
